This Personal Fact Sheet is a great way to organize your personal and financial information for loved ones, or others (conservator, executor or agent) who may need these details should you become incapacitated or pass away. The Fact Sheet includes areas where you can record information regarding your assets and liabilities, any property you own, insurance policies, Advance Health Care Directives, Power of Attorney and Wills.
(Two important points with respect to this process. First, giving a specific person the power to act on your behalf through a Power of Attorney stops with your death. Second, once financial institutions are made aware of a death, individual accounts in the name of the deceased are usually frozen until beneficiaries can be determined. Because of this, if your family members, executor or agent will need access to your money in order to carry out your final wishes, an account in joint name will be helpful.)
